There are two ends of a healthcare system — the blunt end and the sharp end.01:40 – 02:40 The sharp end is where a patient experiences care — where a physician, nurse, or healthcare practitioner is actually delivering treatment. This includes assessments, emergency room visits, triage, and direct interventions.02:40 – 03:11 The blunt end is the system that supports that care — resourcing, policy decisions, infrastructure, and access. Rural vs. urban access is a huge factor. Those in smaller, community-based hospitals often receive different care — not because the doctors are better or worse, but because resources and policy decisions differ.Every healthcare system looks different, and we must recognize where discrepancies start. One key point I emphasize is that cultural competence is competence. We must learn to be culturally competent — not necessarily knowing every detail of a patient’s background, but understanding their story.Just as lawyers tell stories and apply facts to the law, healthcare practitioners must learn the patient’s story to provide proper care. For example, a patient with brown skin reporting pain might be treated differently than a patient with white skin — and that affects health outcomes.Many racialized communities, new immigrants, and those for whom English is not a first language experience mistrust, disbelief, and different tolerance standards for pain. These disparities happen every day.Adverse events — bad outcomes that should not occur — happen in about 8% of all hospital admissions in Canada. What’s worse, marginalized communities disproportionately experience these events.
