In this episode of Supernatural, Sam and Dean are called upon by a former hunter named Martin, who is currently institutionalized, to investigate a case at the mental hospital where he is being held. Martin believes that a mysterious monster is attacking the patients, and he needs the brothers' help to uncover the truth.

To get to the bottom of the case, Sam and Dean pose as patients and get themselves admitted to the hospital. As they begin to investigate, they find themselves struggling with their own inner demons. The stress of their situation pushes them to the breaking point, causing them to unleash their inner turmoil against each other.
