S3·E25 Didn't We Almost Have It All?
Synopsis
The mountain climbers lie about the axe in their friend's head, telling Derek that it accidentally got there. During surgery, he and George discover that it was deliberately put there, and one of the climbers admit that he was trying to "put him out of his misery". Adele is saved after Richard finds her lying in the bathroom and is stabilized, but soon suffers a miscarriage. Richard learns Adele was actually pregnant with his son, not someone she had one night stand with. Bailey is disappointed to see that Callie is the new Chief Resident and begins to question her abilities as a surgeon. Richard chooses Derek as Chief of Surgery, but he refuses, telling him that he isn't done doing what he does best. Cristina and Burke's wedding day arrives, but Cristina won't stay out of the hospital. However, Cristina's nervousness results in Burke's realizing that he does not love the real Cristina and leaves her. Meredith and Derek are uncertain about their future and despite Derek admitting that she is the love of his life, they call their relationship over.

The episode opens with a group of mountain‑climbers arriving at the hospital after a friend is found with an axe lodged in his head. The patients claim the injury was accidental, but during the operation Dr. Derek Shepherd and Dr. George O’Malley discover that the weapon was placed deliberately. One of the climbers confesses that he had intended to “put him out of his misery,” confirming the intentional nature of the trauma.
Meanwhile, Dr. Richard Webber encounters Adele, who is discovered lying in a bathroom. He intervenes, stabilizes her, and she is taken to surgery. Shortly after her operation, Adele suffers a miscarriage. In the aftermath, Richard learns that the pregnancy was his, not the result of a one‑night stand as previously believed, adding a personal dimension to the loss.
At the administrative level, Dr. Miranda Bailey expresses disappointment upon learning that Dr. Callie Torres has been appointed the new Chief Resident. Bailey begins to question Callie’s readiness and competence in the role, setting up tension within the surgical hierarchy.
Later, Richard offers the position of Chief of Surgery to Derek. Derek declines, explaining that he does not feel ready to leave the work he loves most, thereby refusing the promotion.
